The fish is farting

A person in a white coat holds a puffed-up pufferfish by its head, using a small orange tool to repeatedly push it into the fish's open mouth, causing the fish to inflate and deflate slightly with each insertion. The scene takes place in a kitchen with a white cutting board and a knife visible in the background.
